Bring the report and the images
For scans, the written report and the actual images can provide different useful information. Bring the available digital copy or films without delaying necessary care.
Keep the timeline together
Previous prescriptions, laboratory results, discharge summaries and procedure notes help show what has already been tried. Mention medication allergies and any adverse reactions.
You can still attend without a complete file
Do not postpone urgent care to gather records. Explain what is missing; the clinician can advise what is needed. Keep personal records private and avoid uploading sensitive reports to public links.
